This Program aims to reduce the risk of homelessness for people with a mental illness leaving clinical mental health services and reduce the likelihood of people presenting for mental health inpatient care by providing post discharge case management and ongoing support.

Workers are based in Ballarat and Horsham.

Referral to this program is via Clinical Treatment Team, Grampians Psychiatric Services - Clinical Mental Health Services.
