Centacare through its Home Based Outreach (HBOS) and Accommodation Support Program provides support to people in their own home. The program is a psychosocial rehabilitation service which focuses on the development of skills, confidence and strengths for everyday life situations.

People with a mental illness are assisted in gaining greater control of their lives. To encourage a sense of self by assisting with the development of communication and social skills - in addition to the re-development of daily living skills. Similarly, a key focus is the development of positive coping-strategies to maintain healthy home environments. Participants are visited at home and individualised-plans and goals are agreed upon, and set together with the visiting staff.

AIMS

The HBOS program aims to:

• Provide support and rehabilitation to people who have a psychiatric disability
• Promote recovery through information, education and counseling
• Assist in the development of social and independent living skills
• Reduce isolation by helping to link people into a variety of community based social, recreational and/or educational activities
• Help individuals to develop positive coping strategies to better manage their illness and the impact it may have on their lives
• Promote peer support through involvement in shared or group activities
• Help people to achieve their personal goals by providing support and encouragement

Target Group

The service is available to people who:

• Have a diagnosed psychiatric illness, who need support due to their psychiatric disability.
• Live in the Central Highlands area.
• Are aged 16 to 64.
• Satisfy Office of Housing income and asset criteria.

Availability

The Home Based Outreach and Accommodation Support Programs operate Monday to Friday from 9am to 5pm, 52 weeks per year.

Referral is via the Centacare Psychiatric Disability Rehabilitation Support Service Intake system.
