Intensive Homebased Outreach Sometimes referred to as ‘the hidden homeless’, this group of people may be isolated from family and friends, and unable to get access to mental health, housing, or support services. They may reside in boarding, rooming houses, pension level Supported Residential Service or have no fixed abode. We aim to provide a high level of support in order to assist clients to experience stability, re-connect with the community, family and friends, and address personal wellbeing.
Target Group This service is for people who are:
Have a history of homelessness or transience
Availability The Intensive Homebased Outreach Program operates Monday to Friday from 9am to 5pm, 52 weeks per year.
Location This program is available in the following areas: Ballarat 4 - 6 Peel St North, Ballarat VIC 3350
Referral Process Referral is via the Centacare Psychiatric Disability Rehabilitation Support Service Intake system. Referrals can be made by services, consumers and carers.
