Sometimes referred to as ‘the hidden homeless’, this group of people may be isolated from family and friends, and unable to get access to mental health, housing, or support services. They may reside in boarding, rooming houses, pension level Supported Residential Service or have no fixed abode.

We aim to provide a high level of support in order to assist clients to experience stability, re-connect with the community, family and friends, and address personal wellbeing.

Target Group

This service is for people who are:

• Have a psychiatric disability arising from serious mental illness and complex needs
• Aged between 16 and 64.
• From within the Central Highlands area.

Have a history of homelessness or transience

Availability

The Intensive Homebased Outreach Program operates Monday to Friday from 9am to 5pm, 52 weeks per year.

Referral Process

Referral is via the Centacare Psychiatric Disability Rehabilitation Support Service Intake system.
