A program linking people to urgent or essential services who have complex needs, who live in low cost accommodation, who are homeless or at risk of being homeless.

Non-Case Management service linking people with complex needs. who live in low cost accommodation who are homeless or at risk of being homeless to urgent or essential services. The Program uses an Assertive Outreach model to that aims to:

• Assess client needs
• Link clients into mainstream or specialised services.
• Provide assistance to overcome critical episodes of need.
• Assist with innovative responses to unmet client needs.
• Advocacy and short term social support.

Target Group

Complex needs (such as mental illness, intellectual or physical disability, acquired brain injury, frail aged, substance affected).

Live in low cost accommodation (such as Supported Residential Service, Rooming House, Private Hotels) who are homeless or at risk of being homeless to urgent or essential services.

Availability

Ballarat Monday to Friday, 9.00 – 3.30.

Horsham Monday to Wednesday9.00 – 5.00 and Thursday 9.00 – 12.30.

Hopetoun Monday & Tuesday 9.00 – 2.00.
