Community Connections assist people with complex needs living in low cost accommodation or who are homeless or at risk of homelessness, into urgent or essential social and health services.
This outreach service will assess individuals who are experiencing social, emotional and health needs and provide advocacy support and referrals to specialist and community services.
People who suffer a mental illness, have an intellectual or physical disability, an acquired brain injury or are aged who live in a Supported Accommodation Services (SRS), rooming house or private hostel are supported under this program.
Our community connection team also work closely with accommodation providers and the health and welfare sector to develop innovative responses that meet the ongoing needs of target groups.

Individuals, carers, family members, community service organisations and health professionals can make referrals into this program. To lodge a referral please use the Connecting Care portal.
Once a referral has been received our Intake Worker will contact the client to discuss their concerns, needs and how this service can assist.
