Supporting Connections provides both residents of Supported Residential Services (SRS) and their proprietors with access to supports that improve resident’s access to local service networks and builds the skills and capacity of SRS Proprietors and staff in identifying resident needs, improving care planning and better managing challenging behaviours.
The program provides for one on one case management to the SRS residents as required and facilitates improved pathways into mainstream community and health services with a focus on social participation.
The Supporting Connections program aims to improve outcomes for residents’ social and health needs while building access to services.
