PERSONAL HELPERS AND MENTORS SERVICE
Support for those who have functional limitations resulting from a severe mental illness. The service provides increased opportunities for recovery through minimising social isolation and increasing the individual’s connections to the community.
This service aims to enhance an individual’s quality of life, connect people to services and assist with managing daily activities.
The following supports and assistance may be offered depending on individual needs:
- Assistance with managing daily activities including housekeeping, shopping, budgeting and using public transport
- Reconnecting people with their community
- Relationship building
- Referrals to appropriate clinical supports and services

Referrals into this program can be made by individuals, carers, family members, community service organisations and health professionals.
Referrals into this program are via Connecting Care. Once our Worker receives a referral they will contact the individual to discuss how this service can assist.
